    Are you concerned for a family member, friend or neighbour? Do you need help, but don\'t know who to turn to? Contact the Closer-to-Home Team who can lead you in the right direction, whatever your needs. What is Closer to Home? Closer-to-home, set up in Dartmouth in 1992, is a liaison point between health and social services. Its aim is to improve the quality of life for the frail and elderly living in our community. Our goal is to maintain their independence and allow them to remain in their own homes with the provision of packages of care from health, social services, homecare etc. Closer-to-Home is here to co-ordinate these various services and to ensure that each individual client receives the right type and amount of care needed. There are regular weekly Closer-to-Home meetings, joining together members of all health and social services agencies, thereby making everyone aware of any problems there may be with individuals in the community. What Can It Do For Me? There are many ways that Closer-to-Home can help ........ Sometimes a simple thing like shopping can, for some, become an awesome task. One call to Closer-to-Home can help solve the problem. Maybe you have noticed your neighbour is not looking too well and seems to be losing weight - who do you contact without the threat of being \"interfering\"? Telephone Closer-to-Home and the co-ordinator will contact all the relevant agencies on your behalf. Are you caring for someone? Do you need help? Do you know how to get a break yourself? Closer-to Home can put you in touch with the appropriate support that meets your needs.
